OUTLINE OF SERVICE

The Hospital Discharge Service based in Tredegar serves the County of Blaenau Gwent. The Service provides temporary extra help and support for people aged 50 or more on their return home from hospital. This help is provided by Support Workers, who are able to advise on benefits [including form completion], liaise with Social Services and assist with many day-to-day tasks, such as cooking, paying bills and shopping. The Service is usually supplied for up to a maximum of 6 weeks.

The Service is managed by a Part Time Team Leader, with a Part Time Assistant  while the Service is delivered by a team of Support Workers.

Days and Times

The Service operates Monday to Friday from  9.00 until 5.00 pm

The office is open Monday to Friday from  9.00 until 3.00 pm

An answerphone operates at all other times.

Charges The Hospital Discharge Service is provided free
of charge.

Accessibility to disabled:  Office situated on the top floor, via a lift and stairs.
